I hereby give my full consent to Tattoo Studio Darwin to perform a tattoo on my body. In consideration of receiving this tattoo, I hereby release the Tattoo Studio, its employees, artists, and agents from all liabilities, claims, demands, actions, or causes of action in law or equity which I, my heirs, representatives, or assigns may now or in the future have as a result of complying with my request to be tattooed.

I fully understand that any employee or agent of this Tattoo Studio is not acting in the capacity of a medical professional. Any suggestions made by staff are not medical advice and are not a substitute for advice from a qualified medical practitioner.

I understand that I will be tattooed using appropriate techniques, instruments, and pigments. To ensure proper healing, I agree to follow all written and verbal Tattoo Aftercare Instructions provided to me until healing is complete. I understand that healing may take two (2) weeks or longer.

I willingly submit to this procedure with full understanding of possible complications, including but not limited to:

• Infection
• Allergic reaction
• Ink rejection
• Scarring

I acknowledge that neither the artist nor the Tattoo Studio is responsible for the meaning, spelling, or interpretation of any symbol, wording, or design I have chosen. I understand that a tattoo is a permanent change to my body and that no guarantees have been made regarding the ability to remove or alter the tattoo.

I confirm that I am 18 years of age or older.

Answering "YES" does not necessarily prevent you from receiving a tattoo.

I confirm that all information provided by me is true and correct.
I understand this is a legal release form and I agree to be legally bound by it.
